Finding the right fit
Services depend on individual needs, eligibility, authorized plans, staffing, and availability. CIH and Family Supports are Indiana waiver programs; a program reference does not establish eligibility or guarantee a specific service. Golden Judes can discuss its current service scope with you and your case manager.

Questions before you begin

A little clarity goes a long way.

Who can contact Golden Judes about services?

Individuals, family members, guardians, and case managers can contact the team to ask questions and discuss a potential fit.

Do I need to know which service or waiver applies?

No. Start by describing what you are looking for. If you have a case manager or an existing waiver, mention that so the team can discuss the appropriate next step.

Are services available in my location?

Golden Judes serves Northeast Indiana and surrounding communities. Contact the team about your location, the service you need, and current availability.

What should I include in my first inquiry?

Your name, a way to reach you, and a general description of the support you are looking for are enough to begin. Keep medical records, identification numbers, and other sensitive details out of the website inquiry.

Your next step

Start with the person. Build from there.
